When  you order an Angel Light and identify an honoree, the recipient will get a high quality card featuring this year's photo by Jason Weisberger along with a personalized certificate on parchment-style card stock.  Both the card and the certificate are approximately 3.5 by 8.5 inches.

Below is an approximation of the certificate and a copy the form you complete showing how the information is used to create the certificate. The numbers in parentheses do not appear on the actual certificate, but relate to the numbered form sections below.
